Materials Request Form
Guidelines:
- We need 5 week-days notice to pull books.
- Both fiction and non-fiction items.
- For grades Preschool to 8th.
- Items will be checked out for 3 weeks.
- Holds materials 2 days past the pick-up day.
- A maximum of 35 books may be requested.

Assignment Alert Form
Teachers, help the library staff to assist students with special projects and assignments.
When many children in the schools served by Palatine Public Library are working on similar assignments, materials can become scarce. An assignment alert allows the staff to pull appropriate materials and hold them for in-library use for the length of the project.
Please submit this form to Palatine Public Library at least 10 days prior to the date the assignment will be given to the class.
